Creating Change Through Transition-Specific Collaboration Using Person-Driven Planning to Promote Dignity, Advocacy, Determination, and Justice
Discussing the fundamentals of person-driven planning, the presenters will share the state-of-the-art and its connections to human dignity and risk, and the development of self-advocacy and determination for improved outcomes with in practice applications, strategies, and tools to support collaborating for change for working, learning, living, and participating in communities.Participants will be able to: 1. Describe the advantage of person-driven process and identify who drives that process. 2. Determine when a person is ready to work and what makes them ready. 3. Identifying and recognizing the supports of individuals with disabilities and their families and the components of collaboration and how it is used to create change for an individual with a disability.
