Empowering Transitions: A Statewide Model to Enhance post-School Outcomes for Students with Disabilities

Kentucky is transforming postsecondary transition education for students with disabilities with KY Transition 360 for Educators. This session highlights: lessons learned from Year 1 of implementation; share practical tools for assessing implementation fidelity, coaching effectiveness and student outcomes; and actionable strategies to engage non-special education educators in transition professional learning.Participants will be able to: 1. Explain how Kentucky is leveraging a three-tiered model to support district-level implementation of transition strategies. 2. Implement three actionable strategies to engage non-special education educators in transition professional learning within their own professional contexts.
